The Massachusetts Board of Registration in Pharmacy has recently updated Forms and Applications on its website www.mass.gov/dph/boards/ph. Please destroy all prior versions of forms and applications listed below and note that the Board will not accept outdated versions after May 18, 2008.
From the Facilities License/Registration section of the website, download the following
updated applications and forms:
- Change of Manager of Pharmacy or Pharmacy Department
- Relocation of Pharmacy or Pharmacy Department
- Transfer of Ownership for Pharmacy or Pharmacy Department
- Certificate of Fitness
- Store Hours
- Controlled Substance Registration
- Discontinuance of a Community Pharmacy or Pharmacy Department
- Registration to Manage and Operate a New Community Pharmacy
- Petition to Waiver
- Self-Inspection Form for Pharmacy License Renewal
- Continuing Education Form
In regard to Board of Registration in Pharmacy facility applications and forms that request corporation member information; a simple list of names, titles and addresses is sufficient. Protect confidential items such as birthdates and social security numbers by excluding them from your submissions to the Board.
