Transitions of Care: Enhancing the Role of Pharmacist and Improving Patient Outcomes
This activity focuses on the role of the hospital-based pharmacist as part of the health care team. Content will review effective models for transition of care that reduce readmission rates and maximize utilization of pharmacists’ expertise.
Learning Objectives
At the completion of this activity, the participant will be able to:
- Describe the impact of transitions of care on the healthcare system.
- Identify potential barriers that may hinder successful care transitions.
- Explain best practices from the Joint Commission, Centers of Medicare and Medicaid Services (CMS), and various advocacy groups to improve transitional models of care.
- Identify resources to help support pharmacists and pharmacy support staff involved in transitions of care.
Presenter
Ava Messayami, PharmD is a PGY1 pharmacy resident at Mount Auburn Hospital. She received her Doctor of Pharmacy degree from the University of Maryland, Baltimore in May 2021. Dr. Mayssami’s areas of interests include informatics and academia. After completing her PGY-1 residency, she plans to pursue a PGY-2 residency in informatics.
