Licensure & Certification Support

Former students may request licensure support from the Registrar’s Office by completing the licensure request form [PDF]. Licensure applicants should first read the instructions on the state website/application prior to submitting the licensure request to the Registrar’s Office. Questions about licensure applications should be directed to the Board, not the Registrar’s Office.

Note: Effective May 13, 2024, all transcript requests must be submitted through the National Student Clearinghouse. If your licensure application requires a transcript only or a transcript plus a form for completion, please submit your request through the National Student Clearinghouse.

If your licensure request requires only a letter or form from MCPHS, then please proceed in sending your licensure request form. to the Registrar’s Office.
