COVID-19 Vaccine Booster Eligibility

As we are seeing a number of breakthrough cases in those fully immunized due to waning immunity, the CDC has released guidance that all students, faculty, and staff . As a student, faculty, or staff member in an educational setting, you are eligible per the CDC guidelines even if you are not a resident of Massachusetts.

Who is eligible now?

  • 18 years of age or more and completed their primary vaccination series at least six months ago for the Moderna or Pfizer vaccine
  • 18 years of age or more and received their J&J vaccine at least two months ago

Which vaccine should I get?

  • You may choose which COVID-19 vaccine you receive as a booster shot.
  • CDC recommendations allow for mix and match dosing for booster shots (e.g., If your primary series was Pfizer, you may receive Moderna as the Booster).

We highly recommend receiving your booster shot during the Thanksgiving holiday to protect yourself and your loved ones over the upcoming semester break.

 


How to Upload Your COVID-19 Booster Shot Information to CoVerified

Below are step-by-step instructions on how to upload proof of your COVID-19 booster vaccination.

  1. Launch the CoVerified application on your mobile device and select the gray + icon at the bottom of the screen.
  2. From the Add New Report menu, select Upload Vaccine Record.
  3. Complete the Vaccine Record form by first touching the Vaccination Type field and selecting the vaccination type Covid-19 Vaccine.
  4. Complete the next field by touching the Manufacturer field and selecting the appropriate choice based on the vaccination you received. Be sure to choose an option with Booster.
  5. Next, touch the Date Performed field and enter the date you received your COVID-19 booster shot; touch Confirm Date to save your selection.
  6. Lastly, attach a photograph of your COVID-19 Vaccination Record Card for the MCPHS COVID-19 Team to review and approve your record. Touch the Attach a File button followed by the Take a Picture button. To do this, CoVerified may ask for permission to use your camera at this point. Please approve this permission and take a photograph of your card. Please ensure that your first name, last name, and date of birth are clearly written and legible on the front side of your COVID-19 card.
  7. Once you have taken a photograph of your card, you will see the teal Confirm button become available; touch this button to complete your upload.
  8. Do not close the CoVerified application until you receive the SUBMITTED SUCCESSFULLY message.
